Job Description:
As the Supervisor of a clinical team in the Continuity of Care Department, this position is responsible for general oversight of case management staff and processes related to inpatient care of a specifically defined group of patients. Under the direction of the Director, this position makes decisions related to daily case volume and assignments and is responsible for the accuracy and timeliness of documentation related to case management and bedday data.
